Now that you’ve found the best school option(s) for your child, you can Request a School online or by printing and filling out the form here:

The School Request Form can be printed, completed and returned by mail, email, fax or in person to:

As you fill out forms, you will be asked for proof of address and will need to provide:

A current photo ID (driver’s license, state ID, tribal ID, passport or military ID)

AND any combination of TWO of the following items:

Utility bill or start up notice dated within the last 60 days (energy, water, gas, waste)
Mail from any government office (tax return, economic assistance, etc.)
Housing agreement or lease (Note: If you are renting or living with extended family or a friend you must complete the enclosed residency affidavit form, plus one form of proof of address from home owner or renter and two forms in parent/guardian name)
Mortgage Statement or Closing documents dated within the last 60 days.

Other forms:

Primary Care Provider form (PDF): To be filled for students enrolling whose residency will be established with someone other than the biological or adoptive parent.
Residency Affidavit Form (PDF): To be filled out by enrolling students whose parent/guardian is unable to provide proof of residency.
