School Registration Form

Please NOTE: This is not a school request form. This form is for REGISTERING a student who has already been placed at an MPS school. If you need to ENROLL in a school, fill out the School Request Form.

If your child is already placed in an MPS school and you have received placement notification or been asked by your new school to complete online registration, this is the correct form. Before you begin, make sure you have the following information ready:

• Emergency contact information
• Previous school information
• Health/insurance information

Now you are ready to register with your new school for this current school year.

This is a comprehensive registration form that was created to be convenient and accessible to our MPS families, and replaces a number of forms that you would normally need to complete to register at your school. We realize you have already provided some of this information during the school request process, but because of data-privacy we cannot pre-fill the questions in this form, and your school may require other forms as well.
