School request form for the 2022-2023 school year
Thank you for choosing Minneapolis Public Schools! We are glad you are here to request a school, and we look forward to providing your child with a great educational program.
By completing the following steps, you will have the best chance of receiving one of your top choices:
- Carefully complete all steps on this request form.
Please enter up to two schools, and be sure to verify all information before submitting. In some cases you may be asked for proof of address.
To complete school registration this spring, please be prepared to provide the following:
Documentation of your child’s name and birth date
Your child’s immunization record
Your child’s Early Childhood Screening summary (for children entering MPS for the first time requesting High Five or Kindergarten)
List your community school as one of your two listed schools.
If you request only magnet schools, or community schools whose attendance area you don’t live in, unfortunately we won’t be able to guarantee a placement.
Please also note, after February 7, while we will make every effort to honor a request for your community school, we cannot guarantee placement at a specific school.
If your child is not placed at your top requested schools, your child will remain on the waitlist for your top requested school through September 30 or until we are able to offer a spot at the requested school.
